Obtain a signed requisition from your referring healthcare provider. As an OHIP-covered service, cardiology consultations or cardiac tests require a signed requisition from a registered Ontario physician or nurse practitioner.

We will call you directly to confirm your appointment, or we will send the appointment details to the referring healthcare provider.
